 tle Case." This lecture explores the tragic space shuttle disasters of Cha
 llenger (1986) and Columbia (2003) &ndash\; both avoidable accidents that 
 were predicted by engineers but overlooked by management.\nLearn how poor

  warnings led to these catastrophic events. Stephen Carver will unpack the

 ng road (A4174) and enter UWE\, turn right at the roundabout and\nfollow t
 he road to the car parks. Once parked\, use the multiple foot paths to\nwa
 lk to the School of Engineering. The walk is only 5 minutes but if you hav

 Rail\nFilton Abbey Wood train station is about 20-30 minute walk from\nthe
  venue. Turn right when leaving the station and follow the MoD Abbey Wood\
 nperimeter fence to the footbridge into Cheswick Village. Turn left toward
 s the roundabout\,\nthen turn right and walk through the UWE barrier (barr
 ier is for cars\, there is\na footpath to the left-hand side). Next\, turn
  right into a long car park where\nthere is a footpath and bridge in the f
