Description

Our speaker for this event is Richard Scaife (Director, The Keil Centre Ltd). Richard is a Chartered Occupational Psychologist, Chartered Ergonomics and Human Factors Specialist and Director of The Keil Centre Ltd.

Richard has more than 30 years' experience gained in defence, aviation and in the process industries. His experience covers all aspects of human factors, particularly human safety analysis (including human error) and the design and evaluation of equipment to meet user requirements.

In this talk our speaker will cover the following:

  • Introduction to Human Error and how it typically comes about (considering task and environmental factors with a focus on impairment such as fatigue);
  • General description of Human Reliability Assessment (HRA) with generic examples from projects in pre-commissioning, NDT, etc. covering the following steps; Task Analysis; guidewords used: outputs from HRA and their uses
  • Lessons learned from project case studies (anonymous) reducing predicted error rates for different types of work.  This will include radiography, ultrasonic testing, plus other projects outside of the NDT field.
  • Practical control measures that could be used in the future to reduce error rates
